Lake County voters can request a vote-by-mail ballot online, by phone, in person, or with a completed request form. The deadline to request that a ballot be mailed for the November 3 General Election is October 22, 2026.

Read the ballot and return-envelope instructions before you begin.
Fill out your ballot carefully and check that your selections are clear.
Place the ballot in the return envelope and sign where required.
Your completed ballot must be received—not merely postmarked—by 7 p.m. on Election Day.
